This intake and screening form will ask you to listen to an 11 minute audio introduction to my services and provide the numbers you hear throughout the audio. The reason for this is a) As an aptitude test, b) To ensure we feel connected and you know who the person is before you meet me, and c) To support the therapeutic value of the session by exploring our shared views about the significance it has to our wellbeing.
